Welcome to the Fundraiser Application process! We appreciate your interest in organizing a Fundraiser with us. To ensure your application is reviewed and processed promptly, please follow these three essential steps:

Step 1: Application Form Submission. Begin by filling out the form provided below. Please attach your company logo and click the “Submit” button to move to the next step.

Step 2: Fundraiser Agreement Form. You will be directed to the Fundraiser Agreement Form. Please take your time to fill out this form completely and add your electronic signature. Click the “Submit” button to proceed to the final step.

Step 3: W9 Form Submission. After completing the Fundraiser Agreement Form, you’ll be guided to the W9 Form. Fill out all sections of the W9 form thoroughly to ensure accurate processing. Add your electronic signature and click the “Submit” button to complete your application process.
